New Pup Form

Before your dog’s first visit, tell us about them. The form takes about five minutes and covers:

• Your pup — name, age, breed, temperament, medications, and any quirks we should know
• Vaccination proof — upload photos or PDFs of current records (rabies, DHPP, bordetella); 10 MB per file is plenty for phone photos
• Liability waiver — read the current waiver , then check the agreement box and sign on the form

We review everything before your first day and reply if anything’s missing.
