Care Agreement and Liability Release (DRAFT v0)
By signing the intake form, the client (“Owner”) agrees that:
Vaccination requirement. The dog is current on rabies, DHPP, and bordetella vaccinations, and records are on file with Coco’s Canines (“Provider”). The Provider may decline or suspend care for lapsed vaccinations.
Health and behavior disclosure. The Owner has accurately disclosed the dog’s medical conditions, medications, and behavior history, including any history of aggression.
Inherent risk. The Owner acknowledges that group play, off-leash activity, and handling carry inherent risks to dogs and people, including injury and transmission of illness, even with reasonable care.
Release of liability. To the maximum extent allowed by Washington law, the Owner releases the Provider from liability for injury, illness, or death of the dog, or damage to property, except to the extent caused by the Provider’s gross negligence or willful misconduct.
Veterinary authorization. If the dog requires urgent veterinary care, the Provider will make reasonable efforts to contact the Owner and may seek veterinary treatment at the Owner’s expense when the Owner cannot be reached.
Consent to services. The Owner authorizes daycare, boarding, walking, and drop-in care as scheduled, and agrees to applicable rates and cancellation terms as communicated.
Signed electronically via the new pup intake form. The form submission timestamp and signature serve as evidence of consent to this version of the agreement.
